To receive your refund, MAIL in the following:

FYI – I tried this when they ran this promotion a few months ago and they denied my request. I purchased my wipes at Walmart and even though I filled everything out correctly, they returned my rebate to me saying the receipt wasn’t sufficient because it only had the item number and cost and the UPC….but the receipt didnt say “Lysol” wipes. 🙁 I was very diappointed since obviously we have no control over what the store prints on the receipt….
